5.3 Electroconvulsive Therapy: The Most Effective Intervention for TRBD

Despite decades of stigmatization and underutilization, modern ECT (administered under general anesthesia with neuromuscular blockade) remains the most robust evidence-based intervention for severe, treatment-resistant bipolar depression.

The Schoeyen trial. The most rigorous evidence comes from a prospective RCT by Schoeyen and colleagues comparing right unilateral brief-pulse ECT against algorithm-based pharmacotherapy in 73 acutely ill inpatients with TRBD (baseline MADRS 25 or greater) (Schoeyen et al., 2015, American Journal of Psychiatry). The ECT group achieved a response rate of 73.9% compared to 35.0% in the pharmacotherapy group, with a mean MADRS difference of 6.6 points (95% CI 2.5--10.6; p = 0.002). This translates to an NNT of approximately 2.6, though clinicians should note that this estimate derives from a small trial (N = 73) and the confidence intervals around this NNT would be wide.

However, the trial also highlighted the chronicity of TRBD: absolute remission rates did not significantly differ between groups (34.8% ECT vs. 30.0% pharmacotherapy), indicating that while ECT rapidly and powerfully reduces symptom severity, complete remission in multi-refractory patients remains a formidable challenge (Schoeyen et al., 2015, American Journal of Psychiatry).

Meta-analytic confirmation. A systematic review and meta-analysis of 19 studies confirmed robust ECT efficacy in bipolar depression, with pooled response rates of 77.1% (n = 567 bipolar depression patients) and remission rates of 52.3% (Bahji et al., 2019, Acta Psychiatrica Scandinavica).

Bipolar vs. unipolar response. In the same meta-analysis, bipolar depression patients achieved statistically higher response rates (77.1%) and faster speed of response (fewer total sessions needed) compared to unipolar depression patients (74.2%, n = 1,680), with an OR of 0.73 (95% CI 0.56--0.95; p = 0.02) favoring the bipolar group. Remission rates were similar between the two populations (Bahji et al., 2019, Acta Psychiatrica Scandinavica).

Earlier is better. A crucial finding from Swedish national registry data (4,244 patients) demonstrates an inverse relationship between prior pharmacological resistance and ECT response. Response rates decline from 82.1% in patients with zero prior medication failures to 63.4% after five or more failures. Remission rates drop from 34.8% to 13.4% over the same gradient. This evidence argues strongly against the protracted cycling of ineffective oral medications before referring for ECT (Nygren et al., 2023, BJPsych Open).

ECT and suicide risk reduction. A 2025 meta-analysis comprising over 1.18 million individuals (40,867 ECT-treated, 1,141,634 controls across 17 studies) found that ECT was associated with a significant reduction in all-cause mortality (HR = 0.70; 95% CI 0.61--0.81; p < 0.001) and a potent, transient reduction in completed suicide at 3-month follow-up (HR = 0.53; 95% CI 0.39--0.72; p < 0.001) (Rhee et al., 2025, BMJ Mental Health).

5.4 ECT in Specific Bipolar Subtypes

ECT's value is particularly pronounced in three clinical presentations where oral pharmacotherapy is least effective:

Mixed features. In a large naturalistic study of 522 bipolar patients, 72.9% of patients presenting in treatment-resistant mixed states responded to ECT, a rate statistically indistinguishable from the response rate for pure bipolar depression (68.1%) and acute mania (75.0%) (Medda et al., 2015, Journal of ECT). This is clinically significant because mixed states are historically among the most medication-refractory presentations and carry the highest acute suicide risk.

Psychotic features. ECT remains the premier intervention for bipolar depression with psychotic features, where the standard pharmacological approach (mood stabilizer plus antidepressant plus antipsychotic) carries compounding metabolic risks and slow onset.

Catatonia. Catatonia is a severe neuropsychiatric syndrome characterized by motor abnormalities including mutism, rigidity, posturing, and unresponsiveness, sometimes occurring during extreme mood episodes. ECT is the definitive treatment for catatonia that fails to respond to IV benzodiazepines, with response rates of 80.8%, the highest of any bipolar subtype (Medda et al., 2015, Journal of ECT).

5.5 Maintenance ECT

A major limitation of acute ECT is high relapse rates after the treatment course ends. Modern practice strongly advocates for continuation ECT (first 6 months after acute series) and maintenance ECT (extending beyond 6 months). Danish national registry data (19,944 patients treated over two decades) shows that patients receiving continuation/maintenance ECT had significantly lower psychiatric hospitalization rates than those receiving acute ECT only (6-month HR = 0.68; 95% CI 0.60--0.78) (Jørgensen et al., 2024, JAMA Psychiatry).

Maintenance schedules are individualized and symptom-driven, typically tapering from weekly to biweekly to monthly intervals. The combination of maintenance ECT with concurrent lithium appears to synergistically sustain remission gains.

5.6 Cognitive Side Effects: What the Evidence Shows

The most cited barrier to ECT acceptance is concern about cognitive impairment. The evidence requires separating objective from subjective findings:

Objective cognitive testing shows acute, transient declines in processing speed, attention, and verbal memory immediately following an ECT course, but these metrics largely return to baseline (or exceed baseline as the cognitive effects of severe depression resolve) within 2 to 4 weeks after treatment (Semkovska & McLoughlin, 2010, Biological Psychiatry).

Subjective memory complaints persist longer than objective test normalization and diverge based on treatment response: in patients who respond well to ECT, subjective memory complaints gradually improve in parallel with mood recovery. In non-responders, subjective complaints remain entrenched, suggesting they may reflect persistent depressive cognitive symptoms rather than ECT-specific damage.

Modern right unilateral ultrabrief pulse protocols substantially mitigate cognitive side effects compared to the bilateral sine-wave stimulation of earlier decades. Clinicians should counsel patients that some transient memory difficulties are expected during the acute treatment course, that these typically resolve, and that modern techniques have significantly reduced the risk compared to historical ECT.

5.7 ECT vs. Ketamine: Complementary, Not Competing

Intravenous ketamine has emerged as a rapid-acting intervention for severe depression. Multiple head-to-head trials and meta-analyses (2024--2026) have compared it directly with ECT. The synthesized evidence suggests a complementary rather than competitive relationship:

Ketamine produces rapid antidepressant effects (often within hours) but plateaus quickly. ECT builds cumulative efficacy across sequential sessions. Over a typical 4-week acute course, ECT catches up to and surpasses ketamine's effect, with an overall pooled SMD (standardized mean difference, a measure of effect size on the same scale described in Section 2) of -0.59 (95% CI -1.01 to -0.17) favoring ECT after adjusting for baseline severity differences (Rhee et al., 2025, BMJ Mental Health).

The emerging clinical consensus for 2026 positions ketamine as a rapid-acting outpatient tool for moderate-to-severe non-psychotic TRBD, and ECT as the definitive intervention for the most refractory, actively suicidal, psychotic, or catatonic presentations. They are not competitors; they serve different clinical needs at different illness stages.

A practical note on access: ECT requires specialized facilities (anesthesia support, post-procedure recovery space) that are not available at every hospital. In rural and underserved areas, the nearest ECT provider may be hours away. When referring a patient for ECT, clinicians should anticipate the logistical burden on the patient and family (transportation for 3 sessions per week over 3--4 weeks, inability to drive during the treatment course, need for a consistent companion). Ketamine infusion clinics, while also not universally available, are expanding in outpatient settings and may be more geographically accessible for some patients.

Esketamine (intranasal, marketed as Spravato) received FDA approval as monotherapy for unipolar treatment-resistant depression in January 2025 but remains formally off-label for bipolar depression, though clinical use is growing. The pipeline compound NRX-100 (preservative-free IV ketamine) has received FDA Fast Track Designation specifically for severe depression with suicidal ideation, including bipolar populations (NRx Pharmaceuticals, 2024, press release).

Repetitive transcranial magnetic stimulation (rTMS) recently received FDA breakthrough status for bipolar depression. A 2025 meta-analysis of 49 studies and 1,593 patients found active rTMS was significantly superior to sham stimulation (Cohen's d = 0.43; p < 0.001), with rates of treatment-emergent mania equivalent to sham (OR = 1.3; 95% CI 0.7--2.5) and remission rates (27.1%) comparable to those seen in unipolar depression (Ventura et al., 2025, medRxiv preprint; these findings have not yet undergone peer review and the estimates may change following publication). rTMS offers a non-pharmacological option for patients intolerant to medication side effects who do not require the intensity of ECT.
