Section 5: Atypical Antipsychotics in Acute Mania

5.1 Why Antipsychotics Are Now Central to Mania Treatment

Over the past two decades, second-generation (atypical) antipsychotics have moved from adjunctive roles to primary first-line agents for acute mania. This shift is supported by extensive RCT evidence and network meta-analyses demonstrating that SGAs offer rapid onset of action (hours to days versus the 1 to 2 week latency of mood stabilizers), strong efficacy across the severity spectrum, and a generally favorable acute neurological profile compared to first-generation antipsychotics like haloperidol (Cipriani et al., 2011, Lancet; Huang et al., 2024, Molecular Psychiatry).

However, the class is pharmacodynamically heterogeneous. While all SGAs share dopamine D2 and serotonin 5-HT2A receptor activity, their varying affinities for histamine H1, muscarinic M1-M3, and alpha-1 adrenergic receptors create vast differences in metabolic impact, sedation, and movement disorder risk. Selecting the right agent requires a granular, agent-by-agent understanding.

5.2 When to Use an Antipsychotic Alone Versus in Combination

The decision follows a severity-based algorithm.

Monotherapy is appropriate for mild to moderate mania (YMRS scores roughly 12 to 25) without psychotic features. Monotherapy with a first-line SGA yields approximately 50 to 60% response rates (defined as 50% or greater YMRS reduction) within 3 to 4 weeks (Cipriani et al., 2011; Kishi et al., 2022). The advantage is minimizing cumulative side effects.

Combination therapy (antipsychotic plus lithium or divalproex) is the standard of care for severe mania (YMRS above 25), psychotic mania, mixed features, extreme agitation, or a history of partial response to monotherapy. Combination therapy yields approximately 20% higher response rates than monotherapy, with faster trajectory to remission (Tajika et al., 2022). Retrospective hospital data also suggest shorter inpatient stays with combination regimens, though specific effect sizes vary across studies and settings.

5.3 Agent-by-Agent Evidence

Quetiapine (target dose: 400 to 800 mg/day for mania). Quetiapine is unique in having robust evidence across all three bipolar phases: acute mania, acute bipolar depression, and maintenance (CANMAT/ISBD 2018). NNT for acute mania response: 4.2 (95% CI: 2.8 to 8.5), calculated from response rate data in the Kishi et al. (2022) NMA (Kishi et al., 2022, Molecular Psychiatry). Its potent H1 and alpha-1 antagonism produces marked sedation, which is therapeutically useful for restoring sleep in severely agitated inpatients but limits daytime tolerability and long-term compliance. Metabolically, quetiapine carries moderate-to-high risk: average weight gain of approximately 10.5 kg over 52 weeks in the EUFEST first-episode schizophrenia trial (note: this is a schizophrenia population; comparable long-term bipolar-specific weight data are limited, so these figures should be considered approximate for bipolar patients) (Kahn et al., 2008; Fleischhacker et al., 2013), with prominent dyslipidemia, though it is considered slightly less diabetogenic than olanzapine (Pillinger et al., 2020, Lancet Psychiatry). Its "fast-off" D2 binding kinetics give it a near-placebo rate of extrapyramidal symptoms, making it well-suited for patients susceptible to movement disorders.

Aripiprazole (target dose: 15 to 30 mg/day). Aripiprazole is a partial agonist at D2 and D3 receptors with 5-HT1A partial agonism and 5-HT2A antagonism, a fundamentally different mechanism from the pure antagonists (multiple pharmacology sources). NNT for acute mania response: 3.6 (95% CI: 2.5 to 6.1). CANMAT endorses aripiprazole as a first-line maintenance treatment. Its primary advantage is a highly favorable metabolic profile: consistently ranked among the most weight-neutral antipsychotics, with minimal disruption to glucose and lipid parameters (Pillinger et al., 2020, Lancet Psychiatry). The primary limitation is akathisia (a distressing feeling of inner motor restlessness), the most common reason for discontinuation. Clinicians must distinguish akathisia from worsening mania; misdiagnosing akathisia as escalating mania leads to harmful dose increases of the offending agent.

Olanzapine (target dose: 10 to 20 mg/day). Among the most potent antimanic agents available. In the Cipriani NMA, olanzapine produced an SMD of -0.43 (95% CrI: -0.54 to -0.32), and NNT for mania response is 3.8 (Cipriani et al., 2011, Lancet; Kishi et al., 2022). Available as IM injection for emergency tranquilization. However, olanzapine consistently ranks worst-in-class for metabolic burden (Pillinger et al., 2020, Lancet Psychiatry). Its potent H1, 5-HT2C, and M3 antagonism disrupts appetite regulation, impairs insulin secretion, and alters peripheral glucose uptake. Average weight gain: up to 13.9 kg over 52 weeks in the EUFEST first-episode schizophrenia trial (a schizophrenia population; bipolar-specific long-term data are limited but generally consistent with these estimates) (Kahn et al., 2008; Fleischhacker et al., 2013), with over 86% of patients experiencing clinically significant weight gain (7% or greater from baseline) (EUFEST and other long-term trial data). It aggressively drives hypertriglyceridemia, elevated LDL, and rapid-onset insulin resistance. Use is generally reserved for severe, refractory mania unresponsive to metabolically safer agents, and long-term maintenance with olanzapine is increasingly avoided.

Note on funding: Much of the pivotal olanzapine trial data was generated by Eli Lilly, the manufacturer. While the efficacy findings have been independently replicated, the metabolic risk profile was initially underemphasized in manufacturer-supported literature and only became fully apparent through independent post-marketing surveillance and meta-analyses.

Risperidone (target dose: 2 to 6 mg/day). One of the most reliable antimanic agents: NNT of 3.0 (95% CI: 2.1 to 5.7), the lowest in the class (Kishi et al., 2022, Molecular Psychiatry). Pediatric NMAs confirm an SMD of -1.18 in youth, the strongest effect among studied SGAs in that demographic (Vita et al., 2024, Journal of the American Academy of Child and Adolescent Psychiatry). The primary limitation is tight, prolonged D2 binding, which gives risperidone the highest EPS risk among commonly prescribed SGAs, functionally behaving like a typical antipsychotic at doses above 4 to 6 mg/day. Aggressive D2 blockade also causes pronounced, dose-dependent hyperprolactinemia, leading to galactorrhea, gynecomastia, amenorrhea, and sexual dysfunction. Metabolically, risperidone occupies a moderate-risk tier (less than olanzapine, more than aripiprazole).

Ziprasidone (target dose: 80 to 160 mg/day). NNT for acute mania: 3.7 (95% CI: 2.5 to 7.0). Highly weight-neutral with minimal lipid or glucose disruption, making it excellent for patients with metabolic concerns. Low EPS and sedation rates. Two important practical considerations: (1) absorption is food-dependent and requires co-administration with a meal of at least 500 kcal; failure to eat reduces bioavailability by up to 50%, leading to apparent treatment failure that is actually inadequate absorption, and (2) dose-dependent QTc prolongation necessitates a baseline ECG in at-risk populations (multiple prescribing references). Ziprasidone is contraindicated in patients with known prolonged QTc or those on concurrent QTc-prolonging medications.

Cariprazine (target dose: 3 to 6 mg/day for mania; 1.5 to 3 mg/day for bipolar depression). A relatively novel SGA featuring potent D3/D2 partial agonism with approximately 10-fold higher D3 affinity (multiple cariprazine sources). One of very few agents with dual FDA approval for both acute mania and acute bipolar depression. Effect size for acute mania: SMD -0.45 to -0.52; NNT approximately 8 to 12 depending on trial parameters (calculated from response rate data across cariprazine trials; Huang et al., 2024, Molecular Psychiatry). Metabolically favorable: minimal-to-no risk tier for weight gain and metabolic disruption. Primary adverse effects are akathisia (similar to aripiprazole) and dose-dependent EPS, more prominent at the higher antimanic dose range.

Lurasidone: NOT for mania. Lurasidone is FDA-approved for bipolar depression but has no indication, no clinical trial evidence, and no efficacy for acute mania. It must not be prescribed to target manic symptoms. Its bipolar depression efficacy and favorable metabolic profile are discussed in Module 4 (CANMAT/ISBD 2018; lurasidone prescribing information).

Asenapine (target dose: 10 to 20 mg/day, sublingual). Asenapine holds CANMAT first-line status for both monotherapy and combination therapy in acute mania (Yatham et al., 2018, Bipolar Disorders). Its unique sublingual formulation (it is not swallowed; patients must avoid eating or drinking for 10 minutes after administration) reflects its poor oral bioavailability due to extensive first-pass hepatic metabolism. Asenapine demonstrates broad receptor antagonism, including potent 5-HT2A, D2, and alpha-2 adrenergic blockade. Its metabolic profile is moderate: weight gain is clinically significant but generally less severe than olanzapine. The most distinctive side effect is oral hypoesthesia (numbness of the mouth and tongue) from the sublingual administration, which affects approximately 5 to 7% of patients. Akathisia and somnolence are also common. Asenapine has maintenance evidence (preventing manic relapse) and remains a useful option when other first-line agents are not tolerated.

Paliperidone (target dose: 6 to 12 mg/day). Paliperidone, the primary active metabolite of risperidone, holds CANMAT first-line monotherapy status for acute mania (Yatham et al., 2018, Bipolar Disorders). Its pharmacological profile closely resembles risperidone but with a key pharmacokinetic advantage: paliperidone extended-release uses an osmotic delivery system (OROS) that provides more stable plasma concentrations throughout the day, potentially reducing peak-related side effects. Like risperidone, paliperidone carries significant D2-mediated EPS risk and dose-dependent hyperprolactinemia. Its metabolic profile is moderate. An extended-release injectable formulation (paliperidone palmitate) is available and may be relevant for patients with adherence concerns, though the bipolar-specific evidence for the injectable is limited compared to the oral formulation.

Atypical Antipsychotics in Acute Mania: Agent-by-Agent
