This mathematical reality drives the divergence between NICE (which opposes primary care screening) and North American guidelines (which support it). Research evaluating the Number Needed to Screen (NNS) in primary care has found that substantial screening volumes are required to identify even one previously undiagnosed case of a major affective disorder, reflecting the low base rate of bipolar disorder in unselected primary care populations.
