Bipolar disorder shares surface-level features with several psychiatric and medical conditions. Getting the differential wrong leads to inappropriate treatment, from antidepressant monotherapy in unrecognized bipolar patients to unnecessary mood stabilizers in patients with personality pathology. This section addresses each major differential in clinical priority order, emphasizing the specific distinguishing features that drive accurate diagnosis.

5.1 Bipolar Depression vs. Major Depressive Disorder (MDD): The Most

Consequential Differential

The differentiation between bipolar depression and unipolar MDD during a depressive episode is the most consequential decision point in affective psychiatry. The cross-sectional diagnostic criteria for a major depressive episode are virtually identical across both disorders, making longitudinal illness trajectory, specific symptom weighting, and family history the critical discriminators.

**Distinguishing clinical features. **When you are evaluating a depressed patient and trying to determine whether the depression is unipolar or bipolar, several features can tilt the probability. A foundational meta-analysis identified seven clinical characteristics with significant discriminative power for separating Bipolar I depression from MDD: delusions, prominent psychomotor retardation, severe incapacitation, greater mixed symptom burden, higher lifetime number of depressive episodes, shorter individual episode duration, and a history of activating responses to antidepressants. When these seven features are evaluated together using statistical modeling, they correctly classify up to 86.9% of subjects as bipolar or unipolar, a level of accuracy that significantly outperforms any single feature alone.

Bipolar depression frequently presents with "atypical" vegetative features: hypersomnia (excessive sleeping, as opposed to the insomnia typical of MDD), hyperphagia (increased appetite and weight gain), leaden paralysis (a heavy, leaden feeling in the limbs), and extreme mood reactivity. Age of onset provides another clue: bipolar disorder typically emerges in the late teens to early twenties, while the first episode of unipolar MDD often occurs later, in the third or fourth decade.

**Predictors of conversion from MDD to bipolar disorder. **A systematic review and meta-analysis examining the prospective transition from MDD to BD found that approximately 22.5% of individuals initially diagnosed with MDD eventually developed bipolar disorder, with the greatest risk occurring within the first 5 years after MDD diagnosis (Ratheesh et al., 2017, Acta Psychiatrica Scandinavica). This pooled figure combines adult and adolescent samples; conversion rates are likely higher in adolescents, where the first years after an MDD diagnosis carry the greatest risk of bipolar emergence.

The strongest predictors of conversion, expressed as odds ratios (OR; an OR of 2.0 means the factor doubles the odds of conversion, while an OR of 4.0 means it quadruples them):

• Psychotic symptoms during a depressive episode: pooled OR = 4.76 (95% CI: 1.79-12.66) (Ratheesh et al., 2017)

• Family history of bipolar disorder: pooled OR = 2.89 (95% CI: 2.01-4.14) (Ratheesh et al., 2017)

• Earlier age of depression onset: Hedges' g = -0.33 (a standardized measure of difference between groups; here indicating that those who converted to bipolar had meaningfully earlier onset) (Ratheesh et al., 2017)

A separate prospective community cohort study (Pfennig, Ritter, et al., 2016, Acta Psychiatrica Scandinavica) identified additional depressive symptom predictors of subsequent conversion (confidence intervals not reported in the original publication; these point estimates should be interpreted with appropriate caution given the single-study design):

• Diurnal mood variation: OR = 4.30

• Complete anhedonia (loss of pleasure): OR = 2.53

• Feelings of worthlessness and excessive guilt: OR = 2.52

• Increased suicidality: OR = 2.31

**The CANMAT framework. **CANMAT established an influential framework of clinical features "Suggestive of Bipolarity" in a depressed patient. A thorough evaluation should screen for: early onset (<25 years), multiple prior depressive episodes (five or more), postpartum onset, a positive first-degree family history of bipolar disorder, atypical depressive features (hypersomnia, hyperphagia, leaden paralysis), psychotic features or pathological guilt, and a history of antidepressant-induced hypomania, agitation, or rapid cycling (CANMAT/ISBD 2018, Yatham et al., Bipolar Disorders).

**The WHIPLASHED mnemonic. **BAP guidelines use this clinical heuristic (developed by Ronald Pies) for frontline clinicians: Worse in the morning, Hypersomnia, Increased appetite, Psychomotor retardation, Leaden paralysis, Agitated, Substance abuse, Heritability (family history), Early onset, Delusions/Psychosis. The presence of several of these features should maintain a high index of suspicion for bipolarity, even when clear past hypomania cannot be immediately identified (BAP 2016, Goodwin et al., Journal of Psychopharmacology).

Warning: Antidepressant Monotherapy in Unrecognized Bipolar

Depression This differential is not academic. It is the most clinically dangerous diagnostic error in mood disorders. Prescribing antidepressant monotherapy to a patient with unrecognized bipolar depression risks triggering manic switches, accelerating cycle frequency, inducing mixed states, and worsening long-term illness trajectory. All major guidelines (CANMAT, BAP, NICE, VA/DoD) agree: antidepressant monotherapy is contraindicated in Bipolar I and carries significant risk in Bipolar II. Before prescribing an SSRI or SNRI for any patient presenting with depression, the clinician should systematically rule out a history of mania or hypomania.

5.2 Bipolar Disorder vs. ADHD

Bipolar disorder and ADHD share several surface-level features: distractibility, impulsivity, hyperactivity, talkativeness, and difficulty with sustained attention. This overlap makes misdiagnosis common in both directions, with ADHD frequently masking bipolar disorder in youth and young adults. The key to differentiating them is temporality and the specific character of shared symptoms.

**Episodic vs. persistent. **The fundamental distinction is that ADHD is a chronic, persistent, developmental condition present since childhood, while bipolar disorder is an episodic illness characterized by distinct departures from baseline functioning. ADHD symptoms are constant: the patient has always been distractible, impulsive, and hyperactive. Bipolar symptoms emerge in discrete episodes, with a clear before and after.

Specific symptom distinctions:

• Sleep: In ADHD, patients typically have difficulty falling asleep (delayed sleep onset) but feel tired the next day. In mania/hypomania, patients have a genuinely decreased need for sleep: they sleep very little but awaken refreshed and energized.

• Grandiosity: ADHD does not typically involve inflated self-esteem or grandiosity. When present, grandiosity strongly suggests a bipolar component.

• Mood elevation: The "hyperactivity" of ADHD is motor-driven and purposeless. The "hyperactivity" of hypomania is mood-driven and goal-directed, often with a quality of euphoria or expansiveness absent from ADHD.

• Age of onset: ADHD symptoms must be present before age 12 (DSM-5-TR). Bipolar disorder typically emerges in late adolescence or early adulthood, though pediatric presentations exist.

**Objective measurement. **Preliminary research suggests that actigraphy data (wearable movement sensors tracking activity patterns) can help distinguish the two conditions: ADHD shows consistent patterns of excessive daytime motor activity, while bipolar disorder shows marked variability in motor activity patterns correlated with mood state. This remains a research tool rather than a routine clinical application .

**Comorbidity is real. **ADHD and bipolar disorder genuinely co-occur. For a comorbid diagnosis to be valid, ADHD symptoms must persist between mood episodes, not just during mania (when anyone would appear distractible and hyperactive). In practice, this means the clinician should assess the patient during a documented euthymic period: if inattention, impulsivity, and hyperactivity persist when mood is stable, a comorbid ADHD diagnosis is supported. If these symptoms are present only during mood episodes, they are more likely state-dependent manifestations of bipolar disorder rather than a separate diagnosis. Available evidence suggests that stimulant treatment for comorbid ADHD in bipolar patients is acceptably safe when adequate mood stabilizer coverage is in place, though close monitoring for mood destabilization remains essential. 5.3 Bipolar Disorder vs. Borderline Personality Disorder (BPD)

The bipolar-versus-BPD differential is one of the most commonly confused and clinically consequential distinctions in psychiatry. Both conditions involve mood instability, impulsivity, and elevated suicide risk, and the conditions genuinely co-occur more often than chance would predict. However, the treatment implications differ sharply: BPD responds to structured psychotherapy (particularly dialectical behavior therapy), while bipolar disorder requires pharmacological mood stabilization. Misdiagnosing BPD as bipolar disorder exposes patients to unnecessary mood stabilizers and antipsychotics with significant metabolic risk without treating the underlying personality pathology.

**Duration and triggers of mood shifts. **This is the single most reliable distinguishing feature, and it is something you can assess in the clinical interview. Bipolar mood episodes are typically metabolic, autonomous events lasting days to weeks (a minimum of 4 days for hypomania, 7 days for mania, or 2 weeks for depression). BPD mood shifts are reactive, interpersonally triggered (typically by perceived rejection or abandonment), and typically last minutes to hours. Ask the patient: "When your mood shifts, does it usually last hours or days? Is there usually a trigger, like a conflict with someone important to you, or does the shift seem to come on its own?"

**Identity disturbance. **BPD is characterized by a chronic, pervasive disturbance in self-identity, a fluctuating sense of who one is, what one values, and where one belongs. Between mood episodes, patients with bipolar disorder typically have an intact, stable sense of identity. This distinction is especially useful because it is observable between episodes.

**Nature of impulsivity. **In bipolar disorder, impulsivity is state-dependent: it escalates during mania/hypomania and typically normalizes during euthymia. In BPD, impulsivity is trait-based: it is chronic, pervasive, and often tied to emotional regulation difficulties present since adolescence, independent of discrete mood episodes.

**The MDQ problem. **As discussed in Section 3, the MDQ generates false positives at high rates in patients with BPD. Zimmerman and colleagues found that MDQ-positive patients in outpatient settings are equally likely to have BPD as bipolar disorder (Zimmerman et al., 2010/2019, Comprehensive Psychiatry). The specific MDQ items that best discriminate bipolar from BPD are elevated mood, increased goal-directed activity, and episodic co-occurrence, while items related to impulsivity and sexual risk-taking perform poorly as discriminators (Balling et al., 2019).
