2.5 Practical Lithium Management

Baseline workup before the first dose:

Renal function: serum creatinine, BUN, eGFR, urinalysis

Thyroid: TSH (with reflexive free T4)

Serum calcium (adjusted)

ECG for patients over 40 or those with cardiovascular risk factors (lithium can cause benign T-wave flattening)

Pregnancy test for all women of childbearing potential, with contraceptive counseling

Initiation and titration. Standard outpatient initiation begins at 300 mg twice daily (600 mg/day total). For acute inpatient mania, dosing can start at 600 mg two to three times daily (1200 to 1800 mg/day), titrated rapidly by weight and clinical response (multiple prescribing guidelines). Increasingly, clinicians transition patients to a single daily dose given at bedtime, which leverages lower overnight clearance to provide a 12 to 14 hour window where serum levels dip, allowing renal tubular cells a recovery period. This single-dosing strategy reduces polyuria and appears protective against long-term eGFR decline.

Lithium is not metabolized by the liver; it is absorbed orally and excreted 100% intact by the kidneys. Steady-state concentration requires approximately 5 half-lives (5 to 7 days with normal renal function). Serum levels must be checked 5 to 7 days after the first dose and 5 to 7 days after any dose adjustment. All lithium blood samples must be drawn exactly 12 hours (plus or minus 30 minutes) after the last dose to obtain an accurate trough level; drawing during the absorption phase yields falsely elevated readings (multiple pharmacokinetic references).

Target therapeutic levels by illness phase:

Acute mania: 0.8 to 1.0 mEq/L (up to 1.2 mEq/L in severe inpatient cases under close monitoring; adverse effects increase substantially above 0.8)

Standard maintenance: 0.6 to 0.8 mEq/L

Optimized long-term prophylaxis (stable patients, older adults): Some contemporary experts advocate lower target levels of 0.4 to 0.6 mEq/L, with the rationale that this reduces renal and endocrine burden while potentially preserving prophylactic efficacy. The 2025 Chan data showing increased CKD3 risk above 0.58 mEq/L supports this approach from a safety perspective. However, this lower-range strategy has not been validated in prospective controlled trials directly comparing relapse rates or suicide prevention at different maintenance levels, and formal guidelines have not yet adopted it as a primary recommendation. Clinicians using this approach should monitor clinical stability carefully.

What to do when monitoring results are abnormal. The monitoring schedule identifies problems; clinicians also need a framework for responding. If TSH becomes elevated (indicating lithium-induced hypothyroidism), initiate levothyroxine replacement (most PCPs can manage this directly) and continue lithium; hypothyroidism alone is not an indication to discontinue lithium. If eGFR declines progressively (more than 10 mL/min/year or dropping below 60), consult with the prescribing psychiatrist about dose reduction, single nightly dosing (which may be renal-protective), or whether the clinical benefit of lithium justifies continued use with closer renal surveillance. If serum calcium is persistently elevated, evaluate for lithium-associated hyperparathyroidism (PTH level, ionized calcium) and consider endocrinology referral. If the lithium level returns unexpectedly high despite stable dosing, immediately assess for new medications (NSAIDs, ACE inhibitors, diuretics), dehydration, or declining renal function, and hold the next dose pending evaluation. In all cases, coordinate with the prescribing psychiatrist before discontinuing lithium, as abrupt discontinuation carries its own risks (rebound mania, suicidality).

2.6 High-Risk Drug Interactions

Lithium is uniquely vulnerable to dangerous pharmacokinetic interactions because it relies entirely on renal excretion. In the kidney's proximal tubule, transport mechanisms cannot easily distinguish between sodium and lithium ions. Anything that depletes sodium or reduces kidney perfusion forces the kidney to reabsorb more sodium, and lithium gets reabsorbed along with it, driving levels toward toxicity.

Three drug classes are critically dangerous (multiple drug interaction reviews):

NSAIDs (ibuprofen, naproxen, diclofenac). These inhibit cyclooxygenase, reducing renal prostaglandin production. Prostaglandins normally dilate the afferent arteriole feeding the glomerulus; blocking them causes vasoconstriction, sharply decreasing kidney blood flow and lithium excretion. Concomitant use can quickly double serum lithium levels. If mild analgesia is needed, acetaminophen is the safe alternative; it does not affect renal hemodynamics.

ACE inhibitors and ARBs (lisinopril, ramipril, losartan). These alter efferent arteriole tone, decreasing filtration pressure. Population-based studies show that starting an ACE inhibitor in a lithium-treated patient increases the risk of hospitalization for lithium toxicity nearly eightfold within the first 28 days (Juurlink et al., 2004). If antihypertensive treatment is needed, calcium channel blockers (amlodipine) are preferred.

Thiazide diuretics (hydrochlorothiazide, chlorthalidone). These act on the distal tubule, causing sodium and water loss. The resulting volume depletion triggers massive compensatory sodium (and lithium) reabsorption in the proximal tubule. Thiazides are considered contraindicated in lithium patients. Loop diuretics (furosemide) act further down the nephron and theoretically have less impact on proximal lithium reabsorption, but they still cause volume depletion and require weekly lithium monitoring if used.

Important Note: Cross-Disciplinary Drug Interaction Awareness

The most common cause of iatrogenic lithium toxicity is not psychiatric prescribing but medical prescribing. A primary care provider treating a lithium-stabilized bipolar patient for new hypertension or osteoarthritis may not realize that prescribing a standard thiazide, ACE inhibitor, or prolonged NSAID course can precipitate life-threatening lithium toxicity within 2 to 4 weeks. Every non-psychiatric clinician managing a patient on lithium must check for this interaction before adding any new medication. This is not an abstract pharmacology concern; it is one of the most preventable causes of severe harm in bipolar patients.

2.7 Lithium Toxicity: Recognition and Management

Lithium toxicity is a medical emergency that can occur acutely (intentional overdose), chronically (gradual accumulation from declining kidney function, dehydration, or drug interactions), or as an acute-on-chronic event. Because lithium crosses the blood-brain barrier slowly, severe CNS symptoms may lag behind the serum level, meaning clinical presentation must drive intervention rather than the level alone (multiple toxicology sources).

Mild toxicity (1.5 to 2.5 mEq/L): Nausea, vomiting, stomach cramps, diarrhea. The fine resting tremor that is a common harmless side effect worsens into a coarse, debilitating tremor. Lethargy, muscle weakness, lightheadedness.

Moderate toxicity (2.5 to 3.5 mEq/L): Profound mental status changes: confusion, hyperactive agitation, delirium. Motor control breaks down: ataxia (stumbling gait), dysarthria (slurred speech), muscle fasciculations, myoclonus, nystagmus (involuntary eye movements), hyperreflexia.

Severe toxicity (above 3.5 mEq/L): Immediately life-threatening. Grand mal seizures, severe hyperthermia, cardiac arrhythmias, profound hypotension, renal failure, coma, and death.

Management. Any patient with signs of moderate or severe toxicity, or any patient with a level above 1.5 mEq/L with neurological symptoms, requires emergency department evaluation. Immediate actions include stopping lithium and beginning aggressive IV fluid resuscitation with normal saline to restore volume, maximize renal perfusion, and force lithium excretion.

For severe cases, the EXTRIP (Extracorporeal Treatments in Poisoning) Workgroup guidelines provide criteria for hemodialysis (Decker et al., 2015, Clinical Journal of the American Society of Nephrology):

Hemodialysis strongly recommended if kidney function is impaired AND serum lithium exceeds 4.0 mEq/L

Hemodialysis strongly recommended if the patient has decreased consciousness (GCS less than 12), active seizures, or life-threatening cardiac arrhythmias, regardless of the level

Hemodialysis suggested if the level exceeds 5.0 mEq/L (even if asymptomatic) or if the level is not expected to fall below 1.0 mEq/L within 36 hours with fluids alone

Clinical Pearl: Recognizing Toxicity in the Therapy Office

Therapists, social workers, and allied health professionals are often the first to observe a patient across consecutive weeks. If a previously stable patient suddenly presents with clumsiness, stumbling, slurred speech, or a noticeably worse hand tremor, do not assume this is a psychiatric exacerbation or that the patient has been drinking. These are hallmark signs of moderate CNS lithium toxicity. The clinician should immediately halt the session and direct the patient to an emergency department for stat serum lithium and renal function testing. Delay can result in permanent neurological damage.

2.8 Lithium in Pregnancy: Recalibrating the Risk

Lithium was long considered "too dangerous" for pregnancy based on 1970s data from the International Register of Lithium Babies, which suggested a 400-fold increase in Ebstein's anomaly (a rare congenital heart defect). That data suffered from severe ascertainment and recall bias, and modern research has dramatically reduced the estimated risk.

The definitive correction came from Patorno et al. (2017, New England Journal of Medicine), a massive retrospective cohort study of 1,325,563 pregnancies using the U.S. Medicaid database (Patorno et al., 2017, NEJM). Key findings:

Absolute risk of any cardiac malformation: 2.41% in lithium-exposed infants versus 1.15% in unexposed infants

Absolute risk of right ventricular outflow tract (RVOT) obstruction defects (the cardiac malformation category that includes Ebstein’s anomaly): 0.60% versus 0.18% (notably, none of the lithium-exposed cases were specifically coded as Ebstein’s anomaly in the claims data, though most were consistent with defects that frequently co-occur with it)

Adjusted relative risk for cardiac malformations: 1.65 (95% CI: 1.02 to 2.68)

Critically dose-dependent: low doses (600 mg/day or less) carried no statistically significant increase (RR 1.11; 95% CI: 0.46 to 2.64); the association only became significant above 900 mg/day (RR 3.22; 95% CI: 1.47 to 7.02)

The clinical reality is that lithium is a confirmed teratogen, but the magnitude is far smaller than historically cited. The absolute risk increase translates to roughly one additional cardiac malformation per 100 live births. Meanwhile, discontinuing lithium during pregnancy exposes women with severe bipolar I disorder to a high risk of severe mood relapse; prospective and retrospective data consistently show recurrence rates of 50 to 70% during pregnancy and the postpartum period following mood stabilizer discontinuation, with its own downstream risks to both mother and fetus: maternal self-harm, suicide, substance use, and poor prenatal care (Viguera et al., 2000, American Journal of Psychiatry, 157, 179-184; Viguera et al., 2007, American Journal of Psychiatry, 164, 1817-1824).

Current standard of care: lithium remains a viable and often necessary option for pregnant women with bipolar disorder, particularly when compared to the far more teratogenic alternative of valproate (11% major malformation rate). Pregnant women maintained on lithium should undergo high-resolution fetal echocardiography at 16 to 18 weeks gestation. The dose should be kept as low as clinically feasible (below 600 mg/day when possible), lithium levels require more frequent monitoring during pregnancy due to changing renal clearance, and peripartum management must account for the rapid drop in renal clearance postpartum, which can cause levels to spike if the dose is not adjusted (Patorno et al., 2017, NEJM; multiple perinatal psychiatry sources).

The postpartum period carries its own critical considerations. Bipolar women have the highest risk of any psychiatric population for postpartum psychosis (estimated at approximately 20 to 25% in the perinatal psychiatry literature). Observational data suggest that immediate postpartum lithium initiation may reduce this risk substantially, from approximately 25% to approximately 10% (this evidence will be covered in detail in Module 6, but the clinical implication is that lithium should be actively planned for the postpartum period in every bipolar woman, even if it was held during pregnancy).
