3.6 The Child Mania Rating Scale (CMRS)

Adult screening tools are invalid for pediatric populations. The Child Mania Rating Scale-Parent Version (CMRS-P), developed by Pavuluri and Henry (2006, JAACAP), is a 21-item observer-report instrument rated by parents or primary caregivers on a 4-point Likert scale (a rating system where each item is scored on a range, in this case from 1 = "never/rarely" to 4 = "very often") (Pavuluri & Henry, 2006, JAACAP). A validated 10-item brief version also exists.

**Psychometric Properties. **The CMRS-P demonstrates excellent internal consistency and strong test-retest reliability. Receiver operating characteristic (ROC) analysis, a statistical method that evaluates how well a test distinguishes between two groups, yields areas under the curve (AUC, where 1.0 represents perfect discrimination and 0.5 represents chance) of 0.91 to 0.96 for differentiating pediatric mania from ADHD and healthy controls (Pavuluri & Henry, 2006, JAACAP). A total score of 20 or higher flags significant concern.

**Critical item-level interpretation: **Not all high scores mean the same thing. Clinicians should look beyond the aggregate:

• High scores on euphoria (Item 1) and decreased need for sleep (Item 5, distinct from insomnia) are highly specific to bipolar disorder and warrant serious investigation (Pavuluri & Henry, 2006, JAACAP).

• Racing thoughts (Item 8) is universally endorsed in severe PTSD and ADHD and carries low diagnostic specificity for bipolar disorder.

• Hypersexual behavior (Item 13) must prompt immediate screening for sexual abuse before assuming a manic etiology.

The CMRS-P is in the public domain and free for clinical use.

**A note on the Bipolarity Index (BI). **The Bipolarity Index, developed by Aiken and Sachs, is a clinician-rated 100-point diagnostic scale that assesses five domains: symptoms, age of onset, illness course, treatment response, and family history. Unlike the patient-completed screeners discussed above, the BI is filled out by the clinician and provides a dimensional confidence score rather than a binary positive/negative result. A 2022 meta-analysis found the BI to have pooled sensitivity of 0.82 and specificity of 0.73 across studies (Sayyah, Delirrooyfard, & Rahim, 2022, Brazilian Journal of Psychiatry). The BI is most useful as an intermediate tool when screening results are equivocal and a full structured diagnostic interview is not feasible.

3.7 Structured Diagnostic Interviews: The Gold Standard After

Screening

A positive screen on any of these instruments is a gateway to further investigation, not a diagnosis. The gold-standard confirmatory tools are structured diagnostic interviews.

The Structured Clinical Interview for DSM-5 (SCID-5) is the most widely used research-quality diagnostic interview. Its mood disorders module systematically evaluates each DSM-5 criterion for manic, hypomanic, and major depressive episodes, with explicit prompts for duration, functional impairment, and the exclusion of substance-related etiologies.

The Mini International Neuropsychiatric Interview (M.I.N.I.) is a shorter, structured interview designed for clinical rather than research use. It covers the major psychiatric diagnoses with adequate sensitivity and specificity while requiring substantially less administration time than the SCID.

Both instruments reduce the variability of unstructured clinical interviews and ensure that the full DSM criterion set is systematically assessed. For clinicians who do not routinely use structured interviews, the critical takeaway is that screening tools should always be followed by a clinical evaluation that specifically assesses: (1) whether symptoms occurred in discrete, time-bound episodes, (2) the duration and severity of those episodes, (3) the degree of functional impairment, and (4) whether symptoms can be attributed to substance use, medical illness, or personality pathology.

are like a metal detector at the beach: they beep when something might be there, but you still have to dig to find out what it actually is. Sometimes the beep turns out to be a bottle cap rather than a coin. Similarly, a positive bipolar screen can be triggered by anxiety disorders, personality patterns, ADHD, or trauma responses rather than bipolar disorder itself. That is why the follow-up evaluation looks at the whole picture, not just the screening result. And different questionnaires are better at finding different things. Some are better at catching the obvious cases, while others are better at catching the subtle ones. The right choice depends on what the doctor is looking for and who is being screened.
