Section 4: Medical Conditions That Mimic or Exacerbate Anxiety

The DSM-5-TR explicitly requires that a primary anxiety diagnosis is not attributable to the physiological effects of a substance, medication, or another medical condition. Yet diagnostic overshadowing, where somatic symptoms of medical illness are prematurely attributed to a psychiatric disorder, remains a pervasive clinical failure. This section profiles the most important medical mimics, provides evidence for the minimum recommended medical workup, and identifies the clinical red flags that should halt a psychiatric diagnosis and trigger further investigation.

4.1 Endocrine and Metabolic Mimics

Thyroid Disorders

Thyroid dysfunction is the most classical medical mimic of anxiety. Hyperthyroidism (thyrotoxicosis) produces tachycardia, diaphoresis, tremor, weight loss, and psychological hypervigilance that virtually mirror panic disorder and GAD. (StatPearls, Anxiety) However, the clinical picture extends well beyond overt hyperthyroidism.

A systematic review and meta-analysis of 34,094 participants demonstrated that Hashimoto's thyroiditis (autoimmune thyroiditis) is significantly associated with anxiety disorders, with an odds ratio of 2.32 (95% CI: 1.40 to 3.85). (Siegmann et al., 2018, JAMA Psychiatry) Importantly, this association remains significant even in patients who are biochemically euthyroid (normal TSH, normal free T4). A 2024 meta-analysis of euthyroid Hashimoto's patients found 2.52 times higher odds of anxiety compared to controls (OR = 2.52, 95% CI: 1.66 to 3.82), with Beck Anxiety Inventory scores averaging 5.10 points higher. (Romero-Gomez et al., 2024) The leading hypothesis is that circulating thyroid autoantibodies (anti-TPO, anti-TG) may cause direct neuroinflammation independent of peripheral endocrine status.

Clinical implication: Standard anxiety evaluation guidelines recommend TSH and free T4 as the baseline thyroid screen. However, emerging meta-analytic evidence suggests that TPO and TG antibodies may be worth considering in patients with treatment-resistant anxiety and no other identified etiology, particularly given the evidence that euthyroid Hashimoto's patients carry elevated anxiety risk. This testing is not yet part of any major anxiety evaluation guideline but reflects an area of active clinical research.

Pheochromocytoma

Pheochromocytomas are rare neuroendocrine tumors (0.8 to 3 per million person-years) that secrete massive, irregular surges of catecholamines, producing episodes virtually indistinguishable from severe panic attacks. (Saavedra et al., 2024) Autopsy studies suggest that up to 50% of pheochromocytomas are completely unrecognized during the patient's lifetime. Case reports describe patients treated for "treatment-resistant anxiety" for over a decade before a cardiovascular event reveals the tumor. Red flags that should prompt biochemical screening (plasma free or urinary fractionated metanephrines): paroxysmal or treatment-resistant hypertension; panic attacks accompanied by facial pallor rather than flushing; new-onset panic in an older adult without psychiatric history; and paradoxical hypertensive responses to beta-blockers, anesthesia, or tricyclic antidepressants. 4.2 Cardiac Mimics

Supraventricular Tachycardia: The SVT-Panic Overlap

Paroxysmal supraventricular tachycardia (PSVT), particularly atrioventricular nodal reentrant tachycardia (AVNRT), is a frequently misdiagnosed medical condition in outpatient settings. Because episodes are sporadic and self-terminating, standard resting ECGs and 24-hour Holter monitors often fail to capture the arrhythmia.

In a landmark study, 67% of patients eventually diagnosed with PSVT by electrophysiology met full DSM-IV criteria for panic disorder. PSVT remained unrecognized after initial evaluation in 55% of cases, with a median diagnostic delay of 3.3 years. (Lessmeier et al., 1997) After catheter ablation, only 4% of successfully treated patients still met panic disorder criteria, proving that the "panic" was a secondary consequence of the cardiac arrhythmia. A significant gender bias compounds this problem. While 67% of PSVT patients are female, women are twice as likely as men to have their cardiac symptoms attributed to psychiatric causes (65% of women vs. 32% of men). (JAHA, 2020)

Important Note: Palpitations Require Cardiac Investigation

When a patient, particularly a woman, presents with sudden-onset palpitations, do not default to a panic disorder diagnosis without adequate cardiac workup. Standard 24-hour Holter monitors are frequently insufficient for sporadic arrhythmias. Prolonged ambulatory monitoring (14- to 30-day event monitors) should be considered for patients with episodic palpitations and a suspected cardiac etiology. (Lessmeier et al., 1997)

Mitral Valve Prolapse: A Historical Link Now Debunked

Early studies reported elevated MVP prevalence of up to 35% in panic patients. Modern, methodologically rigorous research using blinded evaluations and standardized echocardiographic criteria has largely debunked this causal link. Controlled echocardiographic studies demonstrate no statistically significant difference in MVP prevalence between individuals with panic disorder, social anxiety, and healthy controls. (Filho et al., 2011, British Journal of Psychiatry) The 2020 ACC/AHA Guidelines for Valvular Heart Disease make no mention of anxiety as a feature of MVP. (ACC/AHA 2020 VHD Guidelines) If a patient with known MVP has panic attacks, they should receive standard evidence-based psychiatric treatment or evaluation for a concomitant arrhythmia, rather than assuming the panic is a benign systemic feature of the valve abnormality.

4.3 Respiratory, Vestibular, and Neurological Conditions

Meta-analytic pooled estimates indicate anxiety disorders in approximately 24% of asthmatic patients (CI: 13-41%) and anxiety symptoms in approximately 32% (CI: 22-43%), (Ye et al., 2021, Psychological Medicine) with comorbid anxiety affecting up to 40% of COPD patients. (Willgoss & Yohannes, 2013, Respiratory Care) This relationship is bidirectional: dyspnea triggers the brain's suffocation alarm, and the resulting hyperventilation worsens airway resistance. Comorbid anxiety in COPD patients more than doubles the risk of exacerbation within the first year (HR = 2.10, 95% CI: 1.28 to 3.45). Vestibular disorders produce a significantly elevated prevalence of anxiety compared to the general population, with a relative risk of 1.51 for developing anxiety. (Sciscent et al., 2025, OTO Open) The neural circuitry is direct: vestibular nuclei project to the parabrachial nucleus and amygdala, forming a network critical for fear conditioning. Addressing the underlying vestibular pathology is an important component of resolving secondary anxiety in these patients, alongside standard psychiatric interventions.

Temporal lobe epilepsy (TLE) produces ictal fear that closely mimics panic attacks. The psychiatric comorbidity in pediatric TLE reaches 79%, with anxiety disorders detected in 14% to 23% of cases. Features suggesting a neurological etiology include very brief episodes (under 1 to 2 minutes), associated staring or lip smacking, post-episode confusion, and failure to respond to standard anxiety treatments. 4.4 PANS/PANDAS: An Active Controversy

Pediatric Acute-onset Neuropsychiatric Syndrome (PANS) is defined by the abrupt onset (often within 24 to 48 hours) of OCD or severely restricted food intake, accompanied by severe anxiety, emotional lability, behavioral regression, or other acute neuropsychiatric symptoms. The hypothesized mechanism is a misdirected immune response triggered by Group A Streptococcus (in PANDAS) or other pathogens, leading to basal ganglia neuroinflammation. This remains an active area of controversy. A 2025 AAP Clinical Report acknowledged PANS as "likely a valid diagnosis" but recommended restrictions on diagnostic workups and immunomodulatory treatments. This stance drew immediate criticism from expert consortiums who noted that the report relied on literature searches from 2020 and excluded recent pivotal data, including a Phase III clinical trial that showed 31.1% symptom improvement with IVIG versus 12.1% with placebo on the primary CY-BOCS endpoint, though this difference did not reach statistical significance (p = 0.072). For the practicing clinician, this phenotype, overnight onset of severe OCD with anxiety and behavioral regression in a prepubescent child, warrants careful clinical consideration. The PANS research community recommends specialized immunological workup and multidisciplinary evaluation. The AAP (2025) recommends a more conservative approach, reserving immunomodulatory treatments for specialty centers or clinical trials. Given the active state of this controversy, clinicians encountering this presentation should seek consultation with a center experienced in PANS evaluation to determine the most appropriate workup for the individual patient.

4.5 The Evidence-Based Minimum Medical Workup

Based on AAFP, APA, and AACAP recommendations, the standard initial medical workup for new-onset anxiety includes the following: (AAFP, 2015; StatPearls, Anxiety)

If your doctor orders blood tests or other medical tests as part of an anxiety evaluation, it is because some medical conditions can cause the same symptoms as anxiety: a racing heart, sweating, shakiness, and feeling scared. Thyroid problems, blood sugar changes, and certain heart rhythm issues can all mimic anxiety. These tests help make sure nothing physical is causing or worsening the symptoms, so that treatment can be targeted correctly.

Clinical Pearl: When to Expand the Workup

Expand the medical workup when any of these red flags are present: new-onset anxiety after age 35 with no psychiatric history or psychosocial stressor; prominent somatic symptoms disproportionate to the psychological presentation; panic with facial pallor, treatment-resistant hypertension, or paradoxical medication responses; episodes under 2 minutes with associated confusion or automatisms; overnight onset of OCD with behavioral regression in a child; and complete failure of standard anxiety treatments over 12 or more weeks.

Section 5: Substance-Induced and Medication-Related Anxiety

The DSM-5-TR recognizes Substance/Medication-Induced Anxiety Disorder (SMIAD) as a distinct diagnostic category: prominent anxiety developing during or soon after substance intoxication, withdrawal, or medication exposure. Identifying the offending agent often allows complete symptom resolution without lifelong psychotropic management. This section covers the major substance classes that produce or worsen anxiety, with dose-response data and practical clinical guidance. (DSM-5-TR; APA)

5.1 Caffeine

A 2024 systematic review and meta-analysis of eight RCTs (546 healthy participants) demonstrated that caffeine significantly increases anxiety risk, with a large overall effect size (SMD = 0.94, 95% CI: 0.28 to 1.60). (Liu et al., 2024, Frontiers in Psychology) The dose-response relationship is sharply non-linear. At doses below 400 mg per day, the effect on anxiety is moderate (SMD = 0.61, 95% CI: 0.42 to 0.79, with no significant heterogeneity). At 400 mg per day or above, the effect size escalates dramatically (SMD = 2.86, 95% CI: 2.50 to 3.22), representing a nearly fivefold increase in anxiety impact.

Modern energy drinks, specialty coffee shop brews, and pre-workout supplements can easily exceed 400 mg in a single serving. When a patient presents with new-onset panic attacks or palpitations and daily caffeine intake exceeds 400 mg, the evidence supports reduction as an early intervention (the meta-analytic data shows a nearly fivefold increase in anxiety effect size above this threshold). Observing for 2 to 3 weeks after caffeine reduction before establishing a primary anxiety diagnosis is common clinical practice, though this specific timeline is not formally specified in major evaluation guidelines.

5.2 Cannabis: The THC-CBD Distinction

Anxiety reduction is a frequently cited reason for cannabis use. However, the relationship between cannabis and anxiety is paradoxical and depends entirely on the ratio of cannabinoids. THC (delta-9-tetrahydrocannabinol) produces almost exclusively anxiogenic effects in controlled human studies. In a double-blind crossover study, both 10 mg and 25 mg doses of THC significantly increased anxiety, with the higher dose triggering substantial increases in subjective nervousness. (Spindle et al., 2018) A pooled analysis of four placebo-controlled studies identified a significant sex-by-dose interaction: female participants reported significantly greater anxiety increases than males, even after controlling for body weight and blood THC levels.

CBD (cannabidiol) operates through different mechanisms, including partial 5HT-1A serotonin receptor agonism. Everyday substances can cause or worsen anxiety. Drinking large amounts of caffeine (more than about 3 to 4 cups of coffee a day) can trigger panic-like symptoms in some people. Marijuana, especially products high in THC, tends to increase anxiety rather than reduce it, particularly in people who are not regular users and especially in women. If your doctor suspects a substance might be contributing to your anxiety, they may ask you to reduce or stop using it for a few weeks to see if symptoms improve before starting other treatments.

5.3 Stimulants

A 2025 meta-analysis of 93 RCTs (11,034 patients) confirmed that therapeutic stimulant medications are associated with a significantly elevated risk of overall adverse events compared to placebo (RR = 1.34, 90% CI: 1.27 to 1.41), including anxiety, insomnia, and irritability among the most commonly reported. Illicit stimulant use produces far more severe effects. US hospital admissions for mental health disorders involving methamphetamine use increased 10.5-fold between 2008 and 2020. 5.4 Decongestants: Pseudoephedrine

Pseudoephedrine, widely available over the counter for nasal and sinus congestion, is an indirect sympathomimetic amine that facilitates the release of norepinephrine and epinephrine from presynaptic terminals. The resulting systemic adrenergic surge produces tachycardia, elevated blood pressure, tremors, and jitteriness that are phenomenologically identical to a panic attack. Side effects are dose-dependent: standard therapeutic doses (60 to 120 mg) can induce mild anxiety and insomnia in sensitive individuals, while supratherapeutic doses (180 mg or above) produce severe central nervous system stimulation and potentially dangerous cardiovascular arrhythmias. (BMJ Open Sport & Exercise Medicine, 2016) In older adults and patients with lower urinary tract symptoms, pseudoephedrine contracts bladder neck smooth muscle via alpha-adrenergic receptors, causing urinary retention and dysuria. Because pseudoephedrine is an OTC medication, patients rarely consider it a psychoactive substance and routinely fail to report it during clinical intakes. Clinicians should explicitly ask about cold, sinus, and allergy medication use during psychiatric evaluations. Pseudoephedrine is absolutely contraindicated in patients taking MAOIs due to the risk of fatal hypertensive crisis, and its use should be discouraged in patients with severe panic disorder or uncontrolled hypertension. 5.5 Corticosteroids

Systemic corticosteroids induce neuropsychiatric disturbances in a substantial proportion of patients. In a prospective study of 200 patients, the most common presentations included severe mood disturbances (approximately 17%), anxiety (8%), and psychotic features (5%); (Annals of Indian Psychiatry, 2025; DOI: 10.4103/aip.aip_56_25) broader systematic reviews estimate overall neuropsychiatric rates of approximately 28%, with severe cases at roughly 6%. Management: cautious dose reduction when possible, avoiding abrupt discontinuation (risk of adrenal crisis). Transition to morning-only dosing of short-acting preparations to mimic diurnal cortisol rhythm. If tapering is contraindicated, low-dose atypical antipsychotics or mood stabilizers are second-line options. 5.6 Withdrawal States

Alcohol withdrawal: Mild anxiety and tremor emerge 6 to 12 hours after cessation. Peak danger occurs at 24 to 72 hours (severe anxiety, seizures, delirium tremens). A protracted phase of baseline anxiety may persist for months. Assessment tools include the AUDIT for identifying at-risk drinkers. (Canver, Newman, & Gomez, StatPearls)

Benzodiazepine discontinuation: Produces "rebound anxiety" that transiently exceeds pre-treatment baseline levels. Severity correlates with rate of tapering and the half-life of the agent; short-acting agents like alprazolam produce more severe rebound than long-acting agents like diazepam. (Petursson, 1994) Patients describe the subjective experience as "only fear and anxiety," accompanied by unusual sensory disturbances (tinnitus, internal vibrations). SSRI/SNRI discontinuation syndrome: Captured by the mnemonic FINISH (flu-like symptoms, insomnia, nausea, imbalance, sensory disturbances including "brain zaps," hyperarousal). Onset is rapid (within 2 to 4 days of stopping), and resolution occurs within 1 to 2 weeks, distinguishing it from psychiatric relapse, which develops gradually over weeks. 5.7 Differentiating Primary vs. Substance-Induced Anxiety

The cornerstone of differential diagnosis is the temporal relationship. The DSM-5-TR diagnoses primary anxiety when symptoms clearly precede any substance use, or when symptoms persist for one month or longer after verified abstinence. SMIAD is diagnosed when anxiety developed during or within one month of intoxication or withdrawal, and the implicated substance is pharmacologically capable of producing the observed symptoms. A strong family history of independent psychiatric illness is a key predictor of a primary, endogenous disorder. Patients with substance-induced presentations paradoxically exhibit greater insight into the bizarre nature of their condition and display more acute hostility and physical agitation.
